Requirements to Apply for a Passport

PLEASE NOTE: EFFECTIVE JUNE 2009, U.S. CITIZENS MUST PRESENT A VALID U.S. PASSPORT, U.S. PASSPORT CARD, OR OTHER TRAVEL DOCUMENT APPROVED BY THE DEPARTMENT OF HOMELAND SECURITY, WHEN ENTERING THE UNITED STATES FROM CANADA, MEXICO, THE CARIBBEAN AND BERMUDA AT LAND BORDER CROSSINGS OR SEA PORTS-OF-ENTRY

PROOF OF U.S. CITIZENSHIP

  • Birth certificate with a raised seal and a filing date. (NOT a hospital certificate or photocopy)
    OR
  • Naturalization certificate
    OR
  • Previous U.S. Passport (expired). More than 5 years

(Under age 16: long form birth certificate and expired passport, if applicable)

PROOF OF IDENTITY:

  • Valid driver's License
    OR
  • Military ID
    OR
  • 3 forms of current ID plus bring an identifying witness that has a valid driver's license or a valid passport.

WHO MUST APPEAR:

  • All first time applicants
  • All minors
  • Both parents must give consent for applicants under the age of 16. If one parent cannot appear, that non-appearing parent must give written consent which is notarized
  • One parent must appear with applicant age 16 without a photo ID.

The passport fees for adults eligible to apply by mail using Form DS-82 "Application for a US Passport by Mail" are $130 for the passport book and $30 for the passport card.
Passport Photos are available for $15.00 
National Service Commitment is 25 business days, plus mailing time for routine applications. Expedited passports take approximately 12 business days.
