Personal Care Aide Program and Long Term Home Health Care

This unit coordinates home care services for Medicaid recipients in Sullivan County. 

How to apply?

A licensed physician must submit a Medical Recommendation for Personal Care services to this unit.  A home visit is scheduled and assessment of need is conducted by  a caseworker  from the unit and  a nurse from Public Health Services.  The Departments collaborate  for assessment and provision of aid.  Referral for aides are then processed to private agencies with whom the Department has a contract for services.

This program (as well as the Long Term Home Health Care Program (LTHHCP) enables elderly and disabled persons to remain in their own homes rather than be placed in a care facility. Services are provided through licensed home health care agencies or consumer-directed agencies with which the Department has contracts. Rates for these services are set by the NYS Department of Health.
