Weigh in on Aging in Sullivan County!

Monticello, NY – The Sullivan County Office for the Aging is interested in what residents have to say about what’s important to them as they and their loved ones age in Sullivan County.

A Community Needs Assessment Survey is one of several ways in which the Office is reaching out. The data gathered from this survey is an essential piece of the process that evaluates available resources and supports and identifies gaps in services experience by older adults and their caregivers.

“Information gathered through this survey will become the foundation for the next planning period that runs from 2020-2024,” said Office for the Aging Director Lise-Anne Deoul. “Ideally, the ‘Four-Year Plan’ captures the voices of our public service area and designates funding in the areas most important to the population served by us, so everyone’s responses are very important to us – and will remain strictly confidential. PLEASE take a moment to complete this survey and return it to the Office for the Aging no later than August 19, 2019!”

Anyone can complete the survey at www.surveymonkey.com/r/2LYJWZM, or pick up a copy of the survey at the Office for the Aging (inside the Government Center in Monticello) or a local senior nutrition site.

If you are interested in receiving a number of printed surveys for your group, please contact the Sullivan County Office for the Aging at 845-807-0241.
