Adult Day Health Services

Adult Day Health Care Services located at Sullivan County Adult Care Center

Adult Day Health Care Services is designed to meet the medical, physical, mental, emotional and  social needs of adults in a congregate setting. The program goal is for the registrant remain in the community with the assistance of the family and/or support services as long as their needs can be met and safety maintained. The registrant is linked with needed services. On-site services include nursing, medical social worker, rehabilitation therapies (physical therapy, occupational therapy, and speech therapy), podiatry, nutrition, and a certified nursing assistant.

Hours:

  • Monday through Friday – excludes some holidays - 8:30 AM to 2:30 PM

  • Nursing services
  • Case management and coordination
  • Social services
  • Physical therapy
  • Speech therapy
  • Therapeutic activities
  • Assistance with activities of daily living
  • Blood work and treatments are done as needed per MD orders
  • Dietician

Eligibility Requirements:

  • 21 years of age
  • Medical necessity
  • Adults with limitations

Cost of Service and Fee Structure:

  • Private Pay: $115/day does not include transportation
  • Medicaid: Medicaid rates vary

Method of Payment Accepted:

  • Medicaid or private payments

How to Apply/Documentation Required for Service

  • Call for an appointment for eligibility
  • A person may attend for one day before making a decision to register

Intake Procedure (Application Process)

  • Referral is received
  • A Registered Nurse will assess registrant's needs at the facility and will complete and assessment using the UAS-NY Uniform Assessment System.
  • A physician must sign medical orders
  • If a registrant receives Medicaid, arrangements are made for transportation through Medicaid

  • Availability of days varies

  • There is placement for 17 registrants per day
  • There are a total of 30 registrants that can be enrolled at one time

  • English with Spanish translation available

Physical Accessibility/Proximity to Public Transportation

  • The building is handicapped accessible
  • The Shortline Bus makes stops at the nearby Sullivan County Community Services building
  • The walk to and from the Adult Care Center is hilly and is not recommended for everyone
